As a service to you, we have provided preliminary self assessments that are screening tools that you can complete and have scored here on our website. The purpose is limited to educational and informational purpose’s only and should not to be interpreted as a diagnosis.

The Adult Assessment consists of 24 questions and your response is rated on a scale of zero to five. This is a screening tool for Adult ADD. It is not a diagnostic test. The diagnosis of ADD can only be made on the basis of a detailed history and professional evaluation. Symptoms resembling those of ADD may result from anxiety, depression, mania, and other disorders. These conditions must be ruled out by a professional before a diagnosis of Adult ADD can be made. Neurofeedback can be a very robust tool in re-managing a variety of attention problems.

If prior to taking this self assessment you think you have a 50:50 chance of having ADD, and if you score over 70, you then have a strong probability of having ADD. This assumes that you have had a medical and professional evaluation that has ruled out other causes of attention difficulties.

The Child Assessment consists of seven questionnaires with a checklist for each. Each questionnaire is a brief overview of seven of the most common conditions related to children. Complete one questionnaire at a time. They will be scored separately.

You will receive the best benefit by completing all seven questionnaires but which can be completed at different times to suit your schedule.

A parent or guardian should complete this based on your observations and experience with the child.
